Our Failure Mode and Effects Analysis (FMEA) Excel Template is designed to help you identify possible failures in design, manufacturing or product assembly and is built with structured tables and formatting for ease of use.

FMEA Analysis Excel Template

Start in the FMEA Data sheet and record organizational details such as the Item, Model, Date, Core Team and Prepared By fields.

In the below table, fill out Potential Failure Modes and relevant fields such as Function, Potential Effects of Failure Modes, Potential Causes, Current Process Controls and Recommended Action among others.

Hop over to the FMEA standards sheet and record organizational standards for Severity, Occurrence, and Detection on a scale of 1-10. The Ranking column is color-coded to show any alarmingly high or low levels of those three parameters.

Once you’ve input your standards, you can move back to the FMEA Data sheet and rate the Sev, Occ and Det for each Failure Mode. Fill out the rest of the row for each Failure Mode including Actions Taken and Responsibility and Target Completion Date.

This will help you stay on top of and mitigate the risk of any potential failures in your processes. Preventative maintenance will save you more money and energy than you can imagine.
